(596-26-2-029-0009) Nature and/or Description of the Action Being Approved: Lexington VAMC has a requirement for fungal complement fixation and immunodiffusion testing for fungal diseases.This requirement will be awarded as a sole source firm fixed-priced Blanket Purchase Agreement to Immuno Mycologies Inc., 2701 Corporate Centre Dr., Norman, OK 73069. This testing service is crucial to Veteran care. Total BPA value, see below: Ordering Period 1: 02-19-2026 to 02-18-2027 - $118,845.53 Ordering Period 2: 02-19-2027 to 02-18-2028 - $124,789.38 Ordering Period 3: 02-19-2028 to 02-18-2029 - $131,029.10 Ordering Period 4: 02-19-2029 to 02-18-2030 - $137,582.35 Ordering Period 5: 02-19-2030 to 02-18-2031 - $144,462.46 Total: $656,708.82 Description of Supplies/Services Required to Meet the Agency s Needs: Lexington VAMC, Special Reference Laboratory, provides complement fixation testing (CFT) and Immunodiffusion (ID) for Veterans at this station as well as numerous other stations across the nation. The CFT and ID fungal antigens and positive controls are used to detect antibodies in patient serum by the complement fixation (CF) and immunodiffusion (ID) procedures to aid in the diagnosis of four specific fungal diseases: Histoplasmosis, Blastomycosis, Coccidioidomycosis, and Aspergillosis. It is intended for use in a laboratory environment, near patient or point-of-care setting. These tests are only performed under a physician's order. Description of Efforts Made to ensure that offers are solicited from as many potential sources as possible (including whether a notice was or will be publicized as required by RFO Part 5 and, if not, which exception under RFO Part 5 applies): As this request is being made using simplified acquisition no sources notices will be issued. Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: It is anticipated that the cost to the Government will be fair and reasonable based on market research, previous procurement history and the IGCE. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: Market research was conducted in accordance with Federal Acquisition Regulation Part 10 using System for Award Management (SAM), y, Dynamic Small Business Search (DSBS) and Federal Procurement Data System (FPDS) databases.
