Description
Combine Synopsis/Solicitation # HT940626QE009 is issued as a Request for Quote (RFQ). The DD Form 1449 is being utilized to generate applicable clauses.
The Defense Health Agency, Healthcare Contracting Division- Southeast intends to procure a firm-fixed price contract for Audiometric Booths in support of the Occupational Medicine Clinic at Naval Health Clinic Cherry Point, NC. The contractor shall provide two (2) Audiometric Booths simultaneously, disassemble, remove and dispose of the two (2) existing audiometric booths, complete installation, and validations within seven (7) business days of start of installation for the new audiometric booths. In addition, the vendor shall include audiometric booth freight/shipping, loading and unloading, all labor, booth certification, and sound field calibration. This requirement is being solicited in accordance with Federal Acquisition Regulation FAR Part 12.6; Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services.
