Description
SP Medical Monitoring The Illinois EPA is seeking a vendor to provide provide occupational related medical examinations and certifications. These exams must take place within 60 miles of the IEPA field offices located in Springfield, Rockford, Des Plaines, Champaign, Peoria, Collinsville and Marion. Item # 1: Vendor will provide occupational related medical examinations to selected employees in order to comply with OSHA standards. Vendor shall conduct examinations, evaluate fitness for duty, maintain records, and provide written opinions and reports to IEPA. The line items below represent an estimated number of examinations over the life of the contract (2 years). These numbers are not a guarantee. The vendor will only be paid based on actual usage. Vendor will provide occupational related medical examinations to selected employees in order to comply with OSHA standards.
