NCO 23 contracting office has a requirement for a Single award IDIQ for Emergent First Fill Services at the SFVAHCS Offeror shall provide the following in their proposal: 1. Signed SF 1449 2. Completed price schedule for all LINS. 3. Signed SF30s for any amendments issued 4. Documentation demonstrating vendor s ability to satisfy statement of work. See section B.3 of this solicitation. This includes pharmacy network, Management Oversight Plan and Quality Control Program. 5. A current Drug Enforcement Administration (DEA) license, controlled substance license, and general license from the respective State Board of Pharmacy. All licenses must be in good standing. 6. Documentation showing pharmacists are in good standing with the South Dakota State Board of Pharmacy. Failure to provide this documentation with your proposal will result in the proposal being removed from competition.
