Contractor shall furnish alllabor, equipment, tools, parts, materials, supplies, traffic control, travel, disposal, and incidentals necessary to provide Encampment Site Waste Removal and Cleanup Services in Butte, Colusa, Glenn, Nevada, Sierra, Sutter and Yuba Counties to Caltrans. Bidder shall have at time of bid submittal, continually thereafter, and for the duration of the Agreement, the following: Prime Contractor: A Class A ¿ General Engineering Contractor License and a Hazardous Substance Removal (HAZ) Certification from California Contractor¿s State License Board (CSLB). Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration. Hazardous Materials Transportation License issued by CHP. Trauma Scene Waste Management Practitioner Certificate from State of California Department of Public Health, Medical Waste Management Program. Waste Tire Hauler Registration Decal for each vehicle with effective and expiration dates issued by Cal Recycle. Hazardous Waste Transporter Registration issued by the California Department of Toxic Substances Control. Confined Space Entry Certification, from each employee (in all classifications) as required by the Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA) prior to the start of work. A permit and contractors program documents will also be required prior to the start of work. Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of initial 40 hours of training and annual eight (8) Hour Refresher training from each employee (in all classifications), prior to the start of work. The prime Contractor or subcontractor shall possess at the time of bid submittal, continually thereafter, and for the duration of the Agreement, the following registration, certifications, permits and licenses: Contractor or subcontractor applying, or spraying disinfectant shall possess a Pest Control Business Branch (PCB) or Main (PCM) license. A Qualified Applicator License (QAL) with Category K (health related) staff is required to oversee the business operations prior to the start of work. Both of these licenses are issued by Department of Pesticide Regulation (https://www.cdpr.ca.gov). Contractor or subcontractor applying or spraying disinfectant shall maintain a current and valid Confined Space Entry Certification and Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of initial 40 hours of training and annual eight (8) Hour Refresher training from each employee (see Contractor¿s Personnel Requirements in Exhibit A, Scope of Work), prior to the start of work. Contractor or subcontractor applying, or spraying disinfectant shall also be registered with each individual county agricultural commission where the disinfectant will be applied. Proof of these requirements shall be provided to Caltrans Contract Manager prior to applying or spraying disinfectant. Bidder shall possess at the time of bid submittal, continuously thereafter, and for the duration of the Agreement, a valid and current Motor Carrier Permit issued from the California Department of Motor Vehicles (DMV). Bidder and subcontractor shall possess at the time of bid submittal, continuously thereafter, and for the duration of the Agreement, a valid and current registration with the Department of Industrial Relations (DIR). Caltrans will verify bidder¿s DIR registration; it is not necessary to provide a copy.
