Notice of Intent to Sole Source The Department of Veterans Affairs, Network Contracting Office 04 (NCO4), intends to negotiate on a sole source basis with TransMedics, Inc. in accordance with RFO FAR 12.102(b). The VA Pittsburgh Healthcare System (VAPHS) requires TransMedics, Inc. Organ Care Service (OCS) to provide deceased donor organ recovery, perfusion, and transportation from organ recovery locations within the TransMedics Great Lakes, Mid-Atlantic, Southeast, Northeast, South, Prairies, Midwest, or Texas regions to the VAPHS transplant center in Pittsburgh, PA. The anticipated contract will include a one-year performance period. This notice is not a request for competitive proposals. Responsible sources may identify their interest, accompanied if applicable by supporting literature that clearly demonstrates capability to meet the performance requirements detailed below, to the Contracting Officer no later than 04/14/2026 at 11:00 AM Eastern Time. Telephone responses or inquiries will not be accepted. Determination of procurement strategy based on responses to this notice is solely within the discretion of the Contracting Officer. The NAICS Code for this notice is 492110, Couriers and Express Delivery Services. PERFORMANCE REQUIREMENTS: Contractor shall provide an integrated service that comprises surgical deceased donor liver recovery, normothermic machine perfusion of the donor organ, and transportation of the donor organ to VAPHS. The contractor must have the capability to provide qualified recovery surgeon(s) to travel to the location of identified deceased organ donors within the continental United States and recover the organ immediately upon notification by the VAPHS transplant team of a donor offer, in coordination with the regional Organ Procurement Organization (OPO), and in compliance with Organ Procurement and Transplantation Network (OPTN) protocols. The contractor must possess demonstrated experience coordinating organ transplant logistics with regional OPOs. Upon successful recovery of the organ, the contractor must have air and ground logistics capability to transport the organ to an identified VAPHS operating room in Pittsburgh, PA, while maintaining liver viability through use of a contractor-provided Normothermic Machine Perfusion (NMP) device. The NMP device must comprise preservation and assessment technology that mimics human physiology by artificially perfusing the organ, significantly reducing ischemia, and providing the ability to optimize the organ during transport. The NMP device must preserve organs in an optimal condition when compared to the option of cold storage on ice. Additionally, the contractor must have capability to provide trained Intensive Care Unit/ Critical Care Monitoring (ICU/CCM) registered nurses to maintain the perfusion equipment and monitor organ viability during transportation to the VAPHS transplant center. Upon arrival at the identified VAPHS operating room, the contractor will hand off the viable deceased donor liver suitable for transplant to the VAPHS transplant team. The contractor will not provide direct medical care to VA patients and will not have access to VA Protected Health Information (PHI) or Patient Identifying Information (PII).
