Responders are solely responsible for all expenses associated with responding to this RFI. GENERAL INFORMATION Title of Project: Samsung RS85 Prestige Ultrasound Service Contract Scope of Work: The vendor shall provide maintenance, support, and repair services on all equipment listed below to include preventive maintenance in accordance with the original equipment manufacturer (OEM) written specifications. The vendor shall provide travel, parts, and labor to complete all services covered under this agreement. All parts must be certified to the OEM specifications. The vendor must have an approved MOU/ISA with the Department of Veterans Affairs to provide remote support. The vendor will supply the VA Maryland Health Care System Biomedical Engineering department with a copy of a detailed service report within 2 weeks of maintenance being completed. The service shall be performed during normal business hours Monday through Friday from 8:00am to 4:30 pm unless mutually agreed upon by the contractor and the VA Maryland Health Care System Biomedical Engineering staff. Contractors shall report to Biomedical Engineering in room 5B183 in Baltimore when they arrive on-site before performing any maintenance, scheduled or unscheduled. This equipment will be covered under a support agreement when it represents the best value for the Government. Performance Period: 1 base year and 4 option years beginning at time of award Type of Contract: Preventative and Corrective Maintenance services Place of Performance: Baltimore VA Medical Center 10 N. Greene St. Baltimore, MD 21201 Performance Based Contract: N/A Security: The contractor must have photo ID available when performing work at a VA Medical Center and be willing to present it to VA staff. The vendor is responsible for personal protective equipment. The vendor must have a current Business Associate Agreement (BAA) with the Department of Veterans Affairs to perform maintenance on equipment that would result in incidental access to patient Protected Health Information (PHI)
