QUESTION / COMMENT STATE RESPONSE 1 I see in the documents the total liability is $299,480. Is this for the entire five year contract? This is the estimated 5- year total. 2 What frequency of delivery are you requiring, we are in that area twice per week? Maximum of one time per week, minimum of 2 times per month. 3 Do we drop a pallet and leave or do we need to actually unload and stack? Drop pallet and leave. 4 Is our normal delivery truck acceptable or do we have to send a special independent delivery truck? WMHI has a dock area where the items can be unloaded. It can accommodate most delivery trucks . 5 Is there an incumbent for this contract? If so, what was the award amount? No, this is a new contract. 6 Is a company website mandatory prior to the award notification? Or will this exclude or penalize a company because they are not in full compliance? A company website is not a requirement of this procurement. 7 Is a Certificate of Insurance(COI) needed during the select process? The awardee will need to provide a copy of their COI according to the terms and conditions Section 7.37. 8 How many times month will the supplies be delivered to the Mental Health Institute? See response to question #2. 2. ITB Amendment Effective Date. The revisions set forth herein shall be effective upon release. All other terms and conditions of this ITB not expressly amended herein shall remain in full force and effect.
