OregonBuys# S-44300-00016246 DAS Procurement Services, Version 4.5 March 31, 2023 Page 4 of 73 SECTION 1: GENERAL INFORMATION 1.1 INTRODUCTION The State of Oregon, acting by and through the Oregon Health Authority’s (OHA) Public Health Division (PHD), Family and Child Health Section, (“Agency”), is issuing this Request for Proposal, (RFP), under the provisions of ORS 279B.060 for Family and Informant Interviewer Services for the Maternal Mortality and Morbidity Review Committee (MMRC) (Committee). The Agency is in need of Family and Informant Interviewer services to conduct interviews with families and others (e.g., a diverse array of professionals including medical examiners, coroners, police, social service agencies, hospital staff, community birth workers and other relevant social connections who have knowledge of circumstances surrounding the death of a person that was pregnant at their time of death or within one year of pregnancy) related to the decedents and prepare written narratives and verbal presentations to the Committee about each case that meets program requirements. This position holds a great deal of responsibility to ensure the protection and confidentiality of the information gathered, both in their office environment and in the field. It requires an individual with a health services background (e.g., medicine, nursing, clinical psychology, social work, applied epidemiology) who has had extensive experience and training in conducting structured interviews with persons who have potentially experienced trauma. Additional details on the Scope of the goods or services or both are included in the Scope of Work section. OHA will award one Contract from this RFP with a term of 5 years and a maximum not-to- exceed compensation limit estimated to be $97,500/per year. If a Contract is awarded, OHA shall award a Contract to the highest ranked responsive and responsible Proposer. See Section 5 Proposal Evaluation. Proposer understands and accepts the requirements of this RFP. By submitting a Proposal, Proposer agrees to be bound by terms and conditions in Attachment 5 Sample Contract and as modified by any Addenda, except for those terms and conditions that OHA has reserved for negotiation, as identified in this RFP as a condition to submitting a Proposal for this RFP. 1.2 SCHEDULE The table below represents a tentative schedule of events. SECTION 2: AUTHORITY , OVERVIEW, AND SCOPE 2.1 AUTHORITY AND METHOD Agency is issuing this RFP pursuant to its authority under ORS 279A.050 and ORS 413.033 Agency is using the Competitive Sealed Proposal method, pursuant to ORS 279B.060 and OAR 137-247-0260. Agency may use a combination of the methods for Competitive Sealed Proposals, including optional procedures: a) Competitive Range; b) Discussions and Revised Proposals; c) Revised Rounds of Negotiations; d) Negotiations; e) and Best and Final Offers. 2.2 DEFINITION OF TERMS For the purposes of this RFP, capitalized words are defined in OAR 125-246-0110 or as defined below. Terminology specific to the OregonBuys system may be found at: OregonBuys_ Contract means the contract or contracts awarded from this RFP.
