Question # 1
Following up from the pre -proposal conference just now, could you
please verify that your building at 1888 North Market Street, Frederick
MD 21701 is now open to the public thus allowing us to hand -deliver
our proposal to you should we choose that route ov er using the
mail? Or, allowing a commercial courier such as FedEx to do so?
Section 4.3.1 of the RFPG says we “…are advised to secure a dated,
signed, and time -stamped (or otherwise indicated) receipt of
delivery.” Would this be possible if we chose t o deliver the proposal
by hand ourselves?

Proposals can be hand -delivered to 1888 North Market Street,
Frederick MD 21701 .
Applicants who hand deliver the proposal will be provided with a
receipt of delivery as stated in the Request for Grant Proposals.
Question #2
In Section 2 -Minimum Requirements the RFP states that the applicant
shall provide licenses from the State of Maryland for all Social Workers
providing services. The State Board of Social Workers no longer sends
a hard copy of the license. Is it acceptab le to identify the link for
accessing verification? If this is not an option, is it acceptable to include
copies of completed License Verification Forms that we download from
the link on each Social Worker? ourselves? [sic]

Department of Health we bsite for each Social W orker .
Question #3
The RFP for this program says the transmittal letter should include the Applicant’s DUNS and
SAM numbers.
What is meant by “SAM number”? We have a CAGE code in the System for Award
Management, but I’m not aware of and c an’t find anything specifically called a “SAM
number.” Are you looking for the CAGE code?
