Completed Appendix 1 form must accompany offer to be considered. Lowest bidder must provide proof of requirements prior to award. INITIAL TERM: Implementation Services per the services described in Appendix 1. Implementation support should include approximately 480 hours to ensure the system functions and is utilized as requested. - Offeror Shall met all requirements per the attached Appendix 1.
