Self Insured Medical Plan The Illinois Department of Central Management Services ("CMS") requests proposals from responsive and responsible Offerors to administer a self-insured medical plan(s) including behavioral health services for Enrollees in six (6) distinct benefit plans: the State Quality Care Health Plan (QCHP), Consumer Driven Health Plan (CDHP), Local Care Health Plan (LCHP), Teachers' Choice Health Plan (TCHP), College Choice Health Plan (CCHP), and Local Consumer Driven Health Plan (LCDHP).
