Print Bid Number: WW09-12 Bid Title: Countywide Injection Well Cleaning Project Category: Construction - All Construction Status: Closed Description: ADVERTISEMENT TO BID INJECTION WELL CLEANING PROJECT JOB NO. 09-12 Sealed bids for the following construction project, Injection Well Cleaning Project, Job No. 09-12, will be received by the Bid Receiver, County of Maui, Purchasing Division, Wells Street Professional Center, 2145 Wells Street, Suite 104, Wailuku, Maui, HI 96793, until 2:00 p.m. Hawaii Standard Time, on August 18, 2010. Bids shall be addressed to the Bid Receiver, Finance Department, 2145 Wells Street, Suite 104, Wailuku, Maui, Hawaii and shall be labeled “Injection Well Cleaning Project, Job No. 09-12.” Bids will be opened publicly and read aloud immediately thereafter. Work under this Contract includes furnishing all labor, materials, equipment and services required for the cleaning of eight (8) injection wells for the County of Maui, Hawaii. The work includes furnishing and installing equipment, piping, mechanical well cleaning, chemical well cleaning, site work, and other appurtenances as necessary to complete the work and to provide complete and functional systems constructed in accordance with Contract. Contract Documents and proposal forms may be obtained at the office of the Director of Finance, Purchasing Division, Wells Street Professional Center, 2145 Wells Street, Suite 104, Wailuku, Maui, HI 96793. Compact disks will be available with Contract Documents in electronic (PDF) format. The Contractor must submit pre-qualifications with his bid per the requirements of Specification Section 00100. Work under this Contract shall be performed by a licensed well drilling Contractor who has obtained a C-57 license from the Hawaii Department of Commerce and Consumers Affairs. Two mandatory pre-bid meetings and tours will be conducted for the project. For the first pre-bid meeting and tour, interested parties are to meet at the Wailuku-Kahului Wastewater Reclamation Facility in Maui at 9:00 a.m. on August 4, 2010. For the second pre-bid meeting and tour, interested parties are to meet at the Kaunakakai Wastewater Reclamation Facility at 10:00 a.m. on August 5, 2010. Those who do not attend the pre-bid meetings and register their attendance in writing will not be permitted to bid on the project. The Contractor, and his subcontractor(s), if any, shall comply with all applicable revisions of the law relative to the Prevailing Wage Schedule, as required under Chapter 104, HRS. Bidders are required to comply with newest procurement code, HRS, Chapter 103-D, any rules and regulations and policy directives issued with respect to Chapter 103-D and any amendments thereof. Bidders are requested to submit with their proposal a State and Federal tax clearance certificate. Kalbert K.
