Description
Modification #3 - Addition of Addendum 1 Questions and Responses. All else remains the same. Modification #2 - Correct the deadline listed in VSS. The deadline of March 12, which is listed in the RFP document, is the correct deadline. Modification #1 - Addition of Modified Exhibit B Sample Personal Services Contract to replace Exhibit B Colorado Personal Services Contract Template. All else remains the same. The purpose of this RFP is to solicit responses from qualified Offerors for instrumentation, instrument maintenance plans, analytical methods, and reagents necessary for identifying disorders and confirming second tier test results from newborn dried blood spot specimens for the CDPHE laboratory. This project is expected to be an instrument purchase or instrument reagent loan agreement, wherein, the instrument is provided at no cost other than the purchase of reagents, sample collection devices, controls and maintenance plans. For the purpose of this RFP, this project has been broken into eight (8) workflows, which are identified in the Statement of Work.
