Contexture will provide access to Patient Care 360, the statewide longitudinal health record, to selected county and municipal jails, community providers, and state correctional facilities in accordance with HIPAA and all applicable municipal, state, and federal laws and regulations. Services include onboarding and training of jail and community users; development, enhancement, and maintenance of bi-directional clinical interfaces; access to medication filled data via contracted pharmacy data vendors; care coordination through the Community Resource Network (CRN); transformation services to support workflow and case management; technical assistance; reporting; and program governance support.
