NOTICE OF INTENT TO SOLE SOURCE A 1 YEAR AWARD TO EATON CORPORATION - FOR REPLACEMENT OF UPS BATTERIES FOR THE HUNGTION VA MEDICAL CENTER LOCATED IN HUNTINGTON WV. Eaton Corporation REPRESENTED BY: Lynn Associates, Inc. 8175 Sheridan Drive Buidling #1 Suite #1 Buffalo NY 14221 STATEMENT OF WORK PART A GENERAL INFORMATION A.1 INTRODUCTION The Hershel Woody Williams VAMC located at 1540 Spring Valley Drive, Huntington WV, 25704 has eight (8) Uninterruptable Power Source (UPS) units that allow electricity to be supplied to medical center equipment vital to patient care in the event of normal power disruptions. All 8 units require routine maintenance and testing from factory certified technicians to guarantee reliable alternate power 24/7. EATON UPS MODES have proprietary software that only EATON CORP can service. A.2 BACKGROUND Failure to have reliable UPS units will prevent other necessary patient care as well as increase the risk of catastrophic failure of the sensitive equipment they protect with severe, patient care consequences such as costly down time to MRI, CAT SCAN and IT servers. A.3 SCOPE OF WORK Vendor will provide all labor, material, equipment, tools, and supervision necessary to needs a replace UPS units for the IT and CT departments. Tasks to be accomplished: Provide 40 new Powerware 9390, Serial EE423CAB13; Model 153302085-002 Provide 40 new Powerware 93PM, Serial ES265UXX06; Model 153000034-002 Vendor will provide: New Batteries Removal and EPA approved disposal of old batteries. Installation utilizing proper torque values (where applicable) and verification of new batteries Visually inspect conditions and appearance of the following: Connection terminals inter cell/battery connectors, cables and associated hardware. Cell/battery covers, containers, and post seals. Battery racks or cabinets and associated components and hardware. Cell/battery jar or cover, noting any excessive distortion. Perform cleaning of all accessible surfaces as required. Reporting of Battery Update Visit: The technician(s) will issue the customer a verbal report summarizing the work that was performed, condition of the battery and identifying any critical issues before leaving the customer s site. A Field Activity Report containing applicable notes and observations will be sent within 5 business days. Vendor will provide services within 14 days upon contract award. STATEMENT OF WORK PART C SUPPORTING INFORMATION C.1 Place of Performance Hershel Woody Williams VAMC located at 1540 Spring Valley Drive, Huntington, WV 25704. Delivery vehicles shall keep traffic disruptions to a minimum. Use of any other entry points must be approved by Chief, Engineering Service prior to use. Vendor shall deliver required parts or equipment specified above during approved work times.
