1. Consult with planning team to develop and finalize a participant evaluation survey, including change in practice components; 2. develop and share final evaluation summary, including participant attendance, demographics, practice change reports, and other relevant findings. 3. For summary of activities and minimum qualifications, refer to attached “Scope and Instructions for 2026 Liver Summit Planning”.
-Vendor must be currently registered and compliant in Hawaii Compliance Express (HCE). -Vendor must currently provide educational and clinical support for viral hepatitis to organizations and/or providers. - Vendor must demonstrate experience with planning in-person viral hepatitis educational programs in Hawai'i. -Vendor must provide at least one letter of support from faculty trainer(s) with at least 10 years clinical experience treating patients for viral hepatitis in Hawai'i. -Refer to instructions in attached “Scope and Instructions for 2026 Liver Summit Planning”.
