Description

The purpose of this project is to establish sustainable, outpatient clinic-based non-oncological infusion therapy services through a partnership between Central Health and CommUnityCare. This initiative will expand timely access to medically necessary treatments, improve care coordination across primary and specialty services, and reduce avoidable emergency department utilization for eligible patients. Project success will be defined by the successful implementation of infusion services, reduced wait times, improved patient experience, and a financially viable care model aligned with Central Health’s mission to serve Travis County residents
