Attachment A, are the streets and locations that need permanent patches. Each location will also be identified by pavement classification. The contractor will be responsible in following the current County of Maui standards which includes the cut beyond the temporary patches. All over cuts must be filled with hot crack pour filler. This cost will be included with the area that needs the permanent patch. When needed, the contractor shall replace permanent road striping and raised pavement markers. This cost will be included in the total cost. When needed, the contractor shall provide traffic control. This cost will be included in the total cost. The attached revised proposal will replace all previous proposals in this bid documents and addendums.
